New Patient Registration.

We have an open and expanding list and welcome new patients from anywhere within our practice area. The practice covers Newton Aycliffe, Shildon, Eldon and all the areas in between.

Patients have the right to change GP practice without asking their previous GPs permission. We recommend all new patients have a new patient health check when joining the practice.

Please bring any information on your health that may be helpful to us.

If you wish to register with our practice please come into one of our surgeries, where a receptionist will give you the appropriate forms to fill in and sign.

Medical treatment is available from the date of registration. Please contact reception for further information.

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.

Pre-registration with the practice

When you come to the surgery to register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
